THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
1. Our Commitment to Your Privacy
We are required by law to maintain the privacy of your protected health information (PHI) and to provide you with this notice of our legal duties and privacy practices. We must notify you following a breach of unsecured PHI.
2. How We May Use and Disclose Your Information
Treatment: To provide, coordinate, or manage your healthcare (e.g., sharing info with a specialist).
Payment: To bill and get payment from health plans or other entities.
Healthcare Operations: For office management and quality improvement.
As Required by Law: We may disclose info when required by federal, state, or local law.
3. Special Protections for Substance Use Disorder (SUD) Records
Strict Confidentiality: If we receive or create records protected by federal SUD laws (42 CFR Part 2), we will not disclose them for legal proceedings against you without your specific written consent or a court order.
Single Consent: You may provide a single written consent for all future uses/disclosures for treatment, payment, and operations.
Redisclosure: Once your information is shared as permitted by this notice, it may be subject to redisclosure by the recipient and no longer protected by HIPAA.
4. Your Rights Regarding Your Health Information
Access: You can ask to see or get an electronic or paper copy of your medical record.
Amendment: You can ask us to correct health information you think is incorrect.
Restrictions: You can ask us not to use or share certain health information for treatment, payment, or operations. We are not required to agree, unless you pay for a service out-of-pocket in full and ask us not to share that info with your health plan.
Utah/Idaho Minor Records: Parents and legal guardians generally have full access to a minor’s records under state law (e.g., Utah HB 259), unless specific legal exceptions apply.
